An initial assessment is made at the time of the patient's first appointment. This assessment determines the need for psychiatric intervention, level of services to be provided, and the need for further evaluations.
This initial assessment includes:
- General summary of problem
- Referral source
- Past medical history
- Family/psychosocial history
- Mental status examination
- Current medications/allergies
- Educational needs
- Treatment plan
REASSESSMENT
At each following appointment, a reassessment by the psychiatrist or therapist provides data about treatment progress. This reassessment gathers information about:
- Patient's response to the recommended therapies
- Significant behavioral changes
- Additional patient needs
- Medication therapy and response to medications
- Educational needs
- Family or relationship issues
